Many older LGBTQ+/SGL people who pioneered coming out of the closet are now forced to go back in. As their housing needs and preferences change with age, prejudice is still rampant. And under the U.S. Fair Housing Act, gender identity and sexual orientation are not explicitly protected classes where housing is concerned. Creating appropriate housing for LGBTQ+/SGL seniors must be informed by the life experiences of older people in this community, which often include family rejection, mistreatment, and even violence. This means increasing our cultural competence—from the terminology used, to the design and management of housing developments that consider past traumas. This case study presents innovative housing projects as well as guidance for developers on how to meet some of the unique needs of this particular population.

According to UN-Habitat, the world needs to build 96,000 affordable homes every day to address the global housing crisis by 2030. Yet, better utilizing existing housing stock—through options such as shared housing—can make a significant dent in the need to build more housing. With college students often challenged to find affordable housing and many older adults living alone in homes with spare bedrooms, these two groups are increasingly benefitting from living together. Universities are often well-suited to facilitate students living and learning with older adults in nearby communities. Intentionally fostering intergenerational engagement through places and programs can reduce loneliness, mitigate ageist stereotypes, and help both groups to thrive.

AAAs are the local leaders that develop, coordinate, and deliver a wide range of home and community-based services. These services include information and referral/assistance, case management, home-delivered meals and meals in congregate settings, in-home services, caregiver supports, transportation, evidence based health and wellness programs, long-term care ombudsman programs, and more. People who receive services provided by AAAs have improved health and well-being, helping them remain in their homes and thrive in the community.

By partnering with this community infrastructure, PHAs can improve voucher utilization and leverage and align resources so older adults, people with disabilities, and people experiencing homelessness —all priority populations for federal housing assistance—can obtain supportive services needed to attain housing stability, optimize well-being, and avoid homelessness and costly institutional care. During the COVID-19 pandemic, service coordinators played a pivotal role in the support of older adult residents of publicly funded housing properties. Some independent housing operators employ service coordinators to increase residents’ self-sufficiency, physical security, social connections, and the delivery of long-term community-based supportive services. This report presents results from a survey conducted between June 23 and July 17, 2020 to explore the experiences of these service coordinators during the early months of COVID-19. At the time of the survey, about one-third of respondents were aware of at least one resident on the property who had tested positive for COVID-19. The survey revealed the pandemic’s impact on the lives of older residents of publicly funded housing. Professional support systems that typically provided personal assistance and medical care were interrupted, threatening residents’ physical and mental health. Transportation and resource acquisition systems were also unsettled, creating barriers to activities of independent living such as shopping to acquire food and medication. Social challenges were particularly acute during the early months of the pandemic. Residents demonstrated signs of anxiety and loneliness as their typical experiences of community life were muted. And, while health guidelines and novel benefit programs emerged at a steady clip, communication systems had to be modified from largely in-person formats to accommodate a population of older adults without consistent access to technological platforms.

COVID-19 is disproportionately impacting the health of low income housing residents and minority communities. Those who have symptoms of COVID-19 may not have the knowledge and resources to prevent the spread of infection, to seek appropriate healthcare, and to maintain quarantine. Lack of experience with telemedicine and lack of home blood pressure and glucose monitoring devices will result in worsening chronic disease health outcomes. Furthermore, the increased financial and emotional stress during the COVID-19 epidemic may result in increased need for mental health support. The Bringing Health Home (BHH) Program, a collaboration between the Housing Authority of the City of Austin and the University of Texas Dell Medical School and funding provided by the St. David's Foundation, has trained and hired residents as state-certified Community Health Workers (CHW) to conduct virtual outreach to their neighbors at the largest public housing community to assess and address needs in the context of COVID-19. Using CDC guidelines, the CHWs assess their peers over the phone for COVID-19 symptoms, reinforce preventative measures, link them to testing and medical care, when indicated, and connect them to other available resources, as needed. With this support, it has been possible for BHH residents with COVID-19 to maintain self-isolation and minimize transmission. The BHH team will share the work they are doing to prevent health disparities, minimize COVID-19 deaths, contain the virus, and protect the public welfare by supporting COVID-19 positive households with customized relief packages and symptom monitoring as they remain under quarantine.

From CDC: "Residents in retirement communities and ILF are considered to be at higher risk of severe COVID-19 outcomes because of older age and because they may have underlying health conditions, such as chronic heart disease, diabetes, or lung disease. They also may be at higher risk of getting and spreading the virus because of community characteristics, such as frequent social activities, and shared dining facilities and communal spaces. Guidance specific to retirement and independent living communities can help the residents, and those who help serve them, slow the spread of the virus and prevent serious illness." The CDC guidance includes a checklist for owners and building managers to help protect residents from the spread of COVID-19.

The Centers for Medicare and Medicaid Services (CMS) and states spend over $300 billion per year on the care of dually eligible individuals, yet still do not achieve acceptable health outcomes. In a 2016 study of social risk factors in the Medicare value-based purchasing programs, dual enrollment status was the most powerful predictor of poor outcomes. For example, relative to Medicare-only beneficiaries, dually eligible individuals had 10-31 percent higher risk-adjusted odds of hospital readmission across conditions measured in the Hospital Readmissions Reduction Program, and scores were lower for dually eligible individuals on nearly all (17 of 19) beneficiary-level quality measures in Medicare Advantage.

High-need, high-cost (HNHC) individuals are defined as people of all ages living with clinically complex needs and functional limitations who also incur high health care costs or are likely to do so in the near future. Despite frequent contact with the health care system and substantial medical spending, the physical, social, and behavioral health needs of these individuals often remain unmet due to uncoordinated and fragmented care. Studies suggest that HNHC individuals could benefit from a more holistic approach that coordinates the care they receive and addresses their unmet social needs. Doing so could improve quality of care and quality of life while reducing health care service use
