The Community Transportation Association of America, a national non-profit, will highlight Wheels to Wellness (W2W), a program in Southern Maryland, that allows healthcare staff in rural Maryland to schedule on-demand and pre-scheduled trips using an online portal, and Oklahoma City, OK, who is working with community partners to create a transportation program for parents working towards reunification with their children now in foster care.

Motivated by a desire to improve conditions in neighborhoods similar to the one she grew up in, and guided by resident voices, Williams is working with multi-sector partners to map asthma hospital visits and housing condition data to identify areas in need of support. A growing recognition of the importance of social determinants of health (SDOH) has led to a proliferation of screening and assessment tools, but there are currently no national standards for how to systematically capture and address the non-health needs of patients. To provide some guidance for communities just starting out on this path, All In: Data for Community Health hosted a webinar featuring two subject matter experts.

CLPHA Health Strategic Planning Workshop Presentation

The 2018 CLPHA Health Strategic Planning Workshop, co-facilitated with Health Impact Project (a collaboration of Pew Charitable Trusts and Robert Wood Johnson Foundation) and Grantmakers in Health, convened over 35 communities across the country. Participating teams included internal PHA staff at member agencies who work on health-related initiatives as well as external partners that provide services, education, and other health resources for residents.
