Launched in 2016, the Denver Supportive Housing SIB aims to support residents struggling with homelessness, substance use, and mental health problems by increasing the number of people getting and staying housed and reducing the number of days they spend in jail. The permanent supportive housing model combines a permanent housing subsidy with wraparound services, such as mental health counseling, to help people improve their stability. In Denver, MHCD and the Colorado Coalition for the Homeless (CCH) were selected to offer these services as part of the SIB.

The Home Preservation Initiative (HPI) for Healthy Living seeks to improve asthma outcomes related to unhealthy housing in five neighborhoods in West Philadelphia. By combining home repairs and community health worker home visits, HPI aims to significantly reduce emergency department visits and hospitalizations due to pediatric asthma. For these primarily African-American communities, substandard housing, unemployment, low wages and a lack of education are barriers to the overall health and well-being of residents. Using outcome data, the collaboration will show health care cost savings, aiming to make a strong case for Medicaid reimbursement for home repairs.

The State of Arizona’s Medicaid agency (AHCCCS) recognizes the vital importance of safe, decent and affordable housing to health. With a portfolio of over 3,000 units of affordable housing for Medicaid members with a determination of serious mental illness (SMI) and/or substance use disorder, housing is a major component of how the State of Arizona assists those trying to recover and stabilize.

The Colorado Coalition for the Homeless created the Denver Housing First Collaborative (DHFC) in 2003 with funding provided by a collaboration of federal agencies. The DHFC involved CCH as the lead agency, Denver Department of Human Services (DDHS), Denver Health (DHHA), Arapahoe House, the Mental Health Center of Denver (MHCD) and the Denver VA Medical Center. The DHFC is designed to provide comprehensive housing and supportive services to chronically homeless individuals with disabilities. Initial federal funding created the capacity to house and serve 100 chronically homeless individuals. The program uses a housing first strategy combined with assertive community treatment (ACT) services, providing integrated health, mental health, substance treatment and support services.

Through the coordinated care model, those paying for health care get a better value and health plan consumers get higher quality care at a price we can all afford. And Oregonians are experiencing improved, more integrated care. With a focus on primary care and prevention, health plans and their providers using the coordinated care model are able to better manage chronic conditions and keep people healthy and out of the emergency department.

States are realizing the potential benefits associated with integrating medical care and social services, and are beginning to take the first steps toward developing financing and payment models that encourage this connection. This brief, made possible by The Commonwealth Fund, reviews potential financing mechanisms to facilitate integration, with a particular focus on Medicaid. Drawing from interviews with experts across the country, it offers models ranging from one time seed funding for pilot projects to blended or braided financing arrangements that support comprehensive integration. The brief also highlights payment methodologies designed to influence providers to incorporate social services into their care efforts, with a focus on moving away from fee‐for‐service and toward value based payment strategies like global budgets.

This Issue Brief describes the results of focus groups conducted in six States as part of the Centers for Medicare & Medicaid Services Financial Alignment Initiative to test integrated care and financing models for Medicare-Medicaid enrollees. Five of these States—California, Illinois, Massachusetts, Ohio, and Virginia—are implementing a capitated model demonstration in which Medicare-Medicaid Plans (MMPs) provide coordinated benefits and access to new and flexible services through a person-centered care model. One, Washington, is implementing a managed fee-for-service model demonstration in which health homes are responsible for organizing enhanced integration of primary, acute, behavioral, and long-term services and supports across existing delivery systems for Medicare-Medicaid enrollees and for directing person-centered care for high-cost, high-risk enrollees.

The Denver Social Impact Bond program is an initiative aimed at measurably improving the lives of people most in need by driving resources towards better, more effective programs. Social Impact Bonds are a unique type of performance-based contract where private and/or philanthropic lenders loan funds to accomplish a specific objective and are repaid based on whether the program achieves its goals. Denver’s Social Impact Bond program will use funds from lenders to provide housing and supportive case management services to at least 250 homeless individuals who frequently use the city’s emergency services, including police, jail, the courts and emergency rooms.

The Conway Center is a project of a nonprofit housing and services organization, So Others Might Eat, and a federally qualified health center, Unity Health Care, in Washington, DC. This $90 million community development initiative will colocate employment training, health care services, and affordable housing under one roof in Ward 7, an area of DC experiencing high poverty and unemployment, and poor health outcomes. The partnership aims to improve access to affordable rental housing, increase livable-wage job attainment, and connect residents to high-quality health care services. Although still under construction, this partnership highlights how a shared vision among community-serving organizations and funders can result in a comprehensive strategy for improving resident health and well-being

This brief aims to bring attention to non-Medicaid funding sources that states could potentially blend or braid to address social determinants of health and other needs that are not typically covered by Medicaid. It is intended to familiarize state Medicaid, public health, and other state policymakers with the funding streams of other agencies, and sketch out a continuum of options to help states coordinate funding to better serve the needs of low-income populations. Because this brief focuses on services for adult Medicaid beneficiaries, it does not address many of the funding sources available for children’s services. However, existing efforts to pool funds for children and youth—notably by the Commonwealth of Virginia—could prove instructive for states seeking to launch such an effort for adults.

In December 2016, federal and state policymakers examined the intersection of unstable housing and negative health outcomes at a meeting convened in Washington, D.C., by the National Academy for State Health Policy (NASHP) with support from The Commonwealth Fund. The goal of the meeting was to identify concrete policy recommendations and actionable steps to align health and housing programs to ensure that people with high service needs receive the housing and supportive services they need to become and stay healthy. This report summarizes their findings and recommendations.
