A more regional approach to care is needed – one that involves coordinated, person-centered healthcare with robust connections to social services and community resources. An innovative infrastructure to do just that is underway in four communities across New Jersey: Trenton, Camden, Newark, and Paterson. Efforts begun in 2011 under New Jersey’s Medicaid Accountable Care Organization (ACO) Demonstration Project have evolved into four regional collaboratives that integrate, coordinate, and align all the disconnected programs aimed at making communities healthier.

There’s a growing body of evidence that positively links affordable, stable, and quality housing with improved educational outcomes for children. That research continually points to the positive return on investment for the earliest possible intervention. The Keeping Families Together model turned the usual paradigm for prioritizing affordable housing on its head. Rather than targeting the most “stable” families, Keeping Families Together sought out families with the most complicated cases—those at greatest risk. To equip municipalities with the skills and tools to address these challenges, New York State created the $12 million Cities for Responsible Investment and Strategic Enforcement (Cities RISE) program, which provides leadership, management, and technical support to help 16 municipalities address deteriorating homes, vacant properties, and neighborhood decline through strategic code enforcement. A Summary of Results from the MIHOPE and MIHOPE-STRONG Start Studies of Evidence-Based Home Visiting
A healthy birth and positive experiences in early childhood can promote health and development. One approach that has improved outcomes for children and their parents is home visiting, which provides individually tailored support, resources, and information to expectant parents and families with young children. Health care in the United States is long overdue for an upheaval. The mismatch between costs, by far the highest in the world, and health outcomes, among the worst in the high-income world, has long been glaring.
