The Centers for Medicare and Medicaid Services (CMS) and states spend over $300 billion per year on the care of dually eligible individuals, yet still do not achieve acceptable health outcomes. In a 2016 study of social risk factors in the Medicare value-based purchasing programs, dual enrollment status was the most powerful predictor of poor outcomes. For example, relative to Medicare-only beneficiaries, dually eligible individuals had 10-31 percent higher risk-adjusted odds of hospital readmission across conditions measured in the Hospital Readmissions Reduction Program, and scores were...

The Home Preservation Initiative (HPI) for Healthy Living seeks to improve asthma outcomes related to unhealthy housing in five neighborhoods in West Philadelphia. By combining home repairs and community health worker home visits, HPI aims to significantly reduce emergency department visits and hospitalizations due to pediatric asthma. For these primarily African-American communities, substandard housing, unemployment, low wages and a lack of education are barriers to the overall health and well-being of residents. Using outcome data, the collaboration will show health care cost savings,...

As of 2015, the Centers for Medicare & Medicaid Services (CMS) pays for chronic care management (CCM) services for Medicare beneficiaries with two or more chronic conditions. CMS requires eligible providers to first obtain patients’ verbal (and, prior to 2017, written) consent, to ensure that patients who participate in CCM services understand their rights and agree to any applicable cost sharing. CCM providers must also enhance patients’ access to continuous and coordinated care, including ongoing care management.

With persistent concerns about health care expenditures, the health care field has recognized a group of patients known as super utilizers—people with complex health needs and socioeconomic challenges who have very high levels of hospital use. A well-publicized team-based care management model to address the needs of these patients is the hotspotting model pioneered by the Camden Coalition of Healthcare Providers in New Jersey, first brought to national attention by an article in the New Yorker in 2011. So far, interest in programs to help super utilizers has outpaced the available evidence...

Strengthening primary care is critical to promoting health and reducing costs in the United States. Comprehensive Primary Care Plus, or CPC+, is an advanced alternative payment model for primary care that builds on the foundation of the Comprehensive Primary Care initiative, implemented by the Center for Medicare & Medicaid Innovation from 2012 through 2016. The evaluation is assessing whether CPC+ achieves improved quality, reduced expenditures, and better health for Medicare and Medicaid beneficiaries in thousands of primary care practices using a mixed methods study. We are analyzing...

The number of kids enrolled in Medicaid and the Children’s Health Insurance Program (CHIP) — two government health plans for the poor — fell by nearly 600,000 in the first 11 months of 2018, a precipitous drop that has puzzled and alarmed many health policy analysts, while several states say it reflects the good news of an improving economy.

Improved access to health insurance contributed to reducing worry and stress associated with paying rent/mortgage or purchasing meals among low-income people. Expanding health insurance access may have contributed to increasing the disposable income of low income groups.

While there are many examples of small-scale programs that have integrated care and financing for Medicare-Medicaid eligible individuals, implementation at large scale has been elusive, often limited by concerns that savings will not materialize. The Medicare-Medicaid Coordination Office with its Financial Alignment Demonstration was specifically created to allow states to step forward and develop models that could substantially improve care for beneficiaries while delivering savings to states and the federal programs. We are now six years into this audacious set of pilots, which involve 12...

Medicaid helps low-income seniors, children, people with disabilities, and families get needed health care. Medicaid coverage improves families’ financial security by protecting them from medical debt and helping them stay healthy for work. Medicaid coverage also has long-term health, educational, and financial benefits for children. Click on the map to learn more about Medicaid’s contributions to your state.

Housing and health systems need to work together. Public housing authorities (PHAs) are significant providers of housing to those in need, offering the health sector scale and expertise. Little was known about how PHAs worked with the health sector writ large. With a national survey, we found that PHAs across the country are engaged in a wide range of partnerships with different health organizations that address various target populations and health priorities. Barriers to housing-health collaboration, such as funding and staffing capacity, can be overcome with cross-system partnerships that...

In response to the heightened interest in the relationship between work and the health of individuals and communities, CMCS has clarified that Medicaid funds cannot be used to pay beneficiaries’ wages, but can pay for employment counseling as an optional benefit—to help people get jobs. Years of experience with work requirements for the Supplemental Nutrition Assistance Program, Aid to Families with Dependent Children, and populations with disabilities have developed the evidence for what is needed to help different populations find and keep jobs.

Health and reentry are closely related, and chronic medical, mental health, and substance use problems make it harder for newly released people to seek employment, obtain housing, and avoid reincarceration. Compared with the general population, justice-involved people tend to be in poorer health and need access to physical and behavioral health services, as well as the know-how and motivation to get care.
