Federal, state, and local policies focused on neighborhood improvement have long emphasized the need for community organizations to share information, coordinate activities, and collaborate in the delivery of services. These partnerships build “community capacity,” as a way of promoting local problem solving and community well-being over the longer term. But, there has been only limited research on which patterns of neighborhood networks are most conducive to implementing effective collective work. This report uses social network analysis, drawing from a network survey, and extensive field...

A more regional approach to care is needed – one that involves coordinated, person-centered healthcare with robust connections to social services and community resources. An innovative infrastructure to do just that is underway in four communities across New Jersey: Trenton, Camden, Newark, and Paterson. Efforts begun in 2011 under New Jersey’s Medicaid Accountable Care Organization (ACO) Demonstration Project have evolved into four regional collaboratives that integrate, coordinate, and align all the disconnected programs aimed at making communities healthier.

Grand Rapids, Michigan, is one of the fastest-growing US cities with economic opportunities for businesses. We jumped to the top of polls for the best cities to start a business in 2015 and have maintained top rankings ever since. We also top national lists for best places to call home and raise a family. This does not tell the whole story, though. Communities of color struggle to thrive here. We rank among the worst large US cities for African Americans economically. Almost 40 percent of African Americans in our city live in poverty. They are three times as likely to be unemployed as whites...

The PHNCI Innovation Learning Community grantees spent eighteen months hard at work implementing innovations in their communities. From this community, we have been able to learn what makes an innovation work and be replicable, including leadership buy-in, cross-sector partnerships, and community engagement. In addition to learning about innovation broadly from these grantees, we also want to highlight each grantee’s individual work, including success, challenges, and lessons learned for others who may be interested in replicating their work. To equip municipalities with the skills and tools to address these challenges, New York State created the $12 million Cities for Responsible Investment and Strategic Enforcement (Cities RISE) program, which provides leadership, management, and technical support to help 16 municipalities address deteriorating homes, vacant properties, and neighborhood decline through strategic code enforcement. Cities RISE uses code enforcement strategies to advance broader community development goals, and, in doing so, helps municipalities align programs aimed at improving residents’ quality of life.

With different drivers but a shared set of goals, public health, health care, social services, and other sectors can come together to work upstream and develop creative solutions to solve the complex problems facing communities today. To that end, with funding from the Robert Wood Johnson, the Public Health National Center for Innovations and the Center for Sharing Public Health Services partnered to launch the Cross-sector Innovation Initiative (CSII). The goal of the CSII is to support, promote and disseminate learning about the role of governmental public health departments in aligning...

The Home Preservation Initiative (HPI) for Healthy Living seeks to improve asthma outcomes related to unhealthy housing in five neighborhoods in West Philadelphia. By combining home repairs and community health worker home visits, HPI aims to significantly reduce emergency department visits and hospitalizations due to pediatric asthma. For these primarily African-American communities, substandard housing, unemployment, low wages and a lack of education are barriers to the overall health and well-being of residents. Using outcome data, the collaboration will show health care cost savings,...

After working with 37 communities across the country over the last three years to drive sustainable improvements in community health we’ve we’ve seen how when specific approaches are implemented, namely Bold, Upstream, Integrated, Local, and Data-Driven approaches, meaningful change affecting the health of a community can happen. We’ve done our best to document the processes, the successes, and the failures along the way, and from those real-world experiences we’ve created a set of preliminary tools that can inform and guide those interested in learning about the unique BUILD approach.

Vacant properties, substandard housing, and neighborhood quality profoundly affect our health, education, and safety. Strategic code enforcement programs can serve as communities’ first line of defense for addressing deteriorating homes, vacant properties, and neighborhood decline.
