    The Digital Equity Act (DEA) is a once-in-a-generation opportunity to design systems that will enable true digital equity. Congress and NTIA outlined specifics for states to include in their digital equity plans. The NDIA State Digital Equity Plan Toolkit provides guidance on how to compile the plans. The National Digital Inclusion Alliance (NDIA) team combined our own subject matter expertise, lessons learned from our community of over 1,000 affiliates across the country, and our understanding of the DEA as it appears in the Infrastructure Investment and Jobs Act (IIJA) and Notice of Funding Opportunity into what we hope is a particularly useful toolkit for you, the administering entities, as you embark on this planning process.     A more regional approach to care is needed – one that involves coordinated, person-centered healthcare with robust connections to social services and community resources. An innovative infrastructure to do just that is underway in four communities across New Jersey: Trenton, Camden, Newark, and Paterson. Efforts begun in 2011 under New Jersey’s Medicaid Accountable Care Organization (ACO) Demonstration Project have evolved into four regional collaboratives that integrate, coordinate, and align all the disconnected programs aimed at making communities healthier.

    Moving Health Care Upstream (MHCU) is based on the belief that health systems can address persistent and costly health inequities by moving “upstream”—beyond the walls of hospitals and clinics and into the communities, collaborating with community-based organizations to address the root causes of disease. The various areas of work within MHCU share a common focus-supporting hospitals and community stakeholders in testing and spreading strategies to move upstream, and sharing “what works” to inform the field and accelerate the upstream movement in the field as a whole. Policy Learning Labs are one example of MHCU’s work to spread knowledge and accelerate action in the field.

    The role of public and nonprofit hospitals and hospital systems in supportive housing is not an obvious one at first glance. Traditionally, the role of nonprofit and public hospitals has been to provide primary healthcare and additional health services to the public, often to select “catchment” areas or neighborhoods. The extent of these institutions’ role in housing was limited to either providing a) short-term inpatient beds for medical or treatment services, or b) residences for nurses or other hospital staff. Why then are more and more nonprofit and public hospitals becoming involved in not only creating, but also providing and managing supportive housing?

    Houston children continue to be poisoned by lead even though childhood lead poisoning is considered the most preventable environmental disease among young children. In the past three years, almost 1,200 Houston children between 0 and 72 months old were found to have lead in their blood above the reference value of concern set by Centers for Disease Control and Prevention. The Houston Health Department (HHD) has hosted the Lead-Based Paint Hazard Control Program (LBPHCP) for over 25 years, with the goal of providing a lead-safe home environment through home investigations and home lead abatement.

    The Keeping Families Together model turned the usual paradigm for prioritizing affordable housing on its head. Rather than targeting the most “stable” families, Keeping Families Together sought out families with the most complicated cases—those at greatest risk.     The Home Preservation Initiative (HPI) for Healthy Living seeks to improve asthma outcomes related to unhealthy housing in five neighborhoods in West Philadelphia. By combining home repairs and community health worker home visits, HPI aims to significantly reduce emergency department visits and hospitalizations due to pediatric asthma. For these primarily African-American communities, substandard housing, unemployment, low wages and a lack of education are barriers to the overall health and well-being of residents. Using outcome data, the collaboration will show health care cost savings, aiming to make a strong case for Medicaid reimbursement for home repairs.

    This brief presents a new approach to support the development of early math skills in young children. The approach synthesizes the influence of parents, home environment, and children’s health care providers. The brief draws on research to explain (1) why it is important to support early math development, (2) what early math is (and isn’t), (3) how early math and literacy development intertwine, and (4) the important role parents play in their child’s development. The brief ends with a description of a promising approach to support early childhood math development that leverages communication between parents and health care providers and book sharing during well-child primary care visits.

    Education Leads Home’s State Partnerships on Student Homelessness Project brings together policymakers and practitioners from with the goal of overcoming child and youth homelessness through education. Through the partnership, each state is committed to researching and implementing replicable best practices that address the most urgent needs of their unique homeless student populations. The State Partnerships on Student Homelessness Project is a nonpartisan effort to develop best practices that can be replicated by communities and states nationwide. In its inaugural year of the project, Education Leads Home (ELH) awarded six states – California, Kentucky, Hawaii, Nevada, Oregon, and Washington – small grants through a competitive process. ELH will provide ongoing technical assistance.

    The North Coast Health Improvement and Information Network (NCHIIN) was funded by DASH CIC-START to add new partners, sectors, mental health client summary data, and facility alerts to ACT.md, the care coordination and alerts notification system in Humboldt County, CA. As part of their CIC-START project, NCHIIN developed this document, which provides a methodology for onboarding new organizations, data streams, and sectors into the ACT.md platform.
