CLPHA Housing Is Summit 2022: A Sustainable Approach to Addressing the Social Determinants of Health
The Columbus Metropolitan Housing Authority (CMHA) has formed a partnership with the Healthcare Collaborative of Greater Columbus (HCGC) and their Central Ohio Pathways HUB to provide supportive services to all 14,000 CMHA-assisted households. HCGC’s HUB is a nationally certified program focused on addressing the Social Determinants of Health in an effort to improve health outcomes for at-risk populations. "Residential segregation is at the heart of racial inequality in the country. All of the disparities in the U.S. — in education, in income, wealth, employment, health — between the races are all fundamentally linked to residential segregation. Housing and school segregation function as mutually-sustaining phenomena that limit perceived housing and school choices, constrain social networks, and curb employment and educational potential. Despite the link between housing and school segregation, however, many initiatives combating segregation tend to focus on one or the other instead of recognizing their inherent connectedness.

The Council of Large Public Housing Authorities (CLPHA) hosted The Housing Is Summit in Washington, D.C., on May 3-4, 2018 with 300 partners across the housing, education, and healthcare sectors.
