When Sylvia Blanco and her colleagues from the Housing Authority of the City of Austin were presented with an opportunity to collaborate with UnitedHealthcare Community Plan of Texas, it was an easy decision for them. “We didn’t think twice about the opportunity to partner with UHC,” said Sylvia, their Chief Operating Officer. “The partnership has been tremendous in helping to move the needle on health outcomes for our residents.” Watch this video to learn how UnitedHealthcare Community Plan of Texas is convening with local organizations to implement innovative and timely solutions in Austin.

Play is not only beneficial but essential for people of all ages. Today, there is increasing interest in the benefits of not only play itself, but specifically people from younger and older generations playing together. The built environment in our communities plays a critical role in facilitating this intergenerational engagement and play. Approaches such as Intergenerational Contact Zones (ICZs) introduce strategies for promoting social inclusion and belonging, employing novel ways of planning and designing public spaces to create intergenerationally enriched environments that provide mutual benefits as well as counteract ageism and age-related stereotypes.

Sesame Workshop and Quest Diagnostics as part of the Quest for Health Equity (Q4HE) initiative are working together to create a bilingual (English and Spanish), multimedia program that helps all families build healthy habits as a foundation for lifelong well-being. This effort addresses systemic barriers to health care and helps improve access to the critical resources every family needs to stay well. The latest materials showcase how there are many ways to support families in being healthy in both mind and body. The new resources include two videos and a digital storybook. People with disabilities have a place to turn to find information on COVID-19. They can contact the Disability Information and Access Line (DIAL) to find vaccine locations, make appointments and arrange for transportation. Trained staff at DIAL can also help people with disabilities track down community supports to help with independent living, such as services that help with health care benefits, financial assistance, housing, food and more. DIAL, operated as a collaboration between a consortium of organizations serving people with disabilities and USAging, is funded by the Administration for Community Living. Staff at DIAL can also make referrals to local disability organizations. For more information, watch this short informational video.

In 2020 we launched a dedicated effort to learn more about legal issues surrounding unaccompanied minors experiencing homelessness. This project was intended to guide both organizations’ ongoing work and advocacy and develop resources to help the field better prevent and end homelessness among minors. This toolkit includes: • Key issues and challenges for minors experiencing homelessness; • Strategies and lessons learned from advocacy for state minor consent to services laws (including questions to consider); • Legal issues and considerations relevant to host homes for minors; • Working towards equity while serving minors; • Child welfare and youth homelessness; and • Additional legal and policy issues.

Head of Strategic Initiatives and Resource Development, Catherine Crago, discussed how their nonprofit subsidiary, Austin Pathways, assists with blending and braiding financing, including corporate and philanthropic funding and in-kind gifts. This webinar is part on an ongoing series sponsored by the Housing Is Working Group. It is a collective of public housing authorities and non-profit organizations working in health, education, and housing sectors coming together monthly via virtual meetings and other forums to discuss cross-sector work and challenges.

The Vancouver Housing Authority collaborated with a Federally Qualified Health Center and a homeless crisis response system to develop a network of scattered-site and site-based supportive housing. This moderated discussion will cover how VHA paired Housing Choice Vouchers and public housing with a Medicaid-funded supportive housing benefit to serve people identified by the community’s Coordinated Entry as needing supportive housing. Speakers will also discuss the challenges faced through the process, model adjustments made, and evaluation of the work through matching housing data and Medicaid utilization data.

The Columbus Metropolitan Housing Authority (CMHA) has formed a partnership with the Healthcare Collaborative of Greater Columbus (HCGC) and their Central Ohio Pathways HUB to provide supportive services to all 14,000 CMHA-assisted households. HCGC’s HUB is a nationally certified program focused on addressing the Social Determinants of Health in an effort to improve health outcomes for at-risk populations. The HUB oversees 10 Care Coordination Agencies (CCAs) and their employed Community Health Workers (CHWs) to provide these services. The HUB contracts with Medicaid Managed Care Organizations, who provide payments to the HUB for services called Pathways delivered to their members.

The Administration for Community Living’s Aging and Disability Network is a multifaceted service infrastructure for older adults and people with disabilities so they can find housing and obtain services like chore assistance, delivered meals, and transportation. By partnering with this community infrastructure, PHAs can improve voucher utilization and leverage and align resources so older adults, people with disabilities, and people experiencing homelessness —all priority populations for federal housing assistance—can obtain supportive services needed to attain housing stability, optimize well-being, and avoid homelessness and costly institutional care. Public and affordable housing agencies and organizations have the potential to serve as a crucial foundation for promoting early school success for economically challenged, fragile and otherwise marginalized children and families through a 24/7/365 multigenerational system of support and an ability to meet families 'where they are.' During this engaging session, Housing Is and our long-time partner The Campaign for Grade-Level Reading will elevate the critical importance of these housing-education partnerships, with special attention to the way in which such collaborations begin. This plenary will explore initial partnership development work taking place in Kansas City, MO, one of the 20 finalist communities recently announced for the 2022 All-America City Awards. The theme of this year's award is "Housing as a Platform to Promote Early School Success and Equitable Learning Recovery." Attendees have the unique opportunity to hear from a partnership in its earlier stages of development and learn alongside the speakers how critical cross-sector work can be launched.

The Centers for Disease Control and Prevention found that Black women are three times more likely to die from pregnancy related causes than white women. With the majority of PHA residents being Black, Indigenous, or people of color, PHAs understand they play a critical role in addressing racial inequities through increased focus on maternal health. This plenary, announced as part of the Biden-Harris Administration’s Maternal Health Call to Action, will discuss the role of PHAs in supporting the health and well-being of their residents.
