With their unique role at the intersection of population health data and technology, Health Information Exchanges (HIEs) are supporting innovative and feasible approaches to enabling better linkages between healthcare and social service providers across the country. The past several years have seen increased attention to the impact which the Social Determinants of Health (SDoH) have on individual and community health outcomes with issues such as housing, food security, education, transportation, and safety strongly impacting a community’s ability to help residents thrive. Healthcare and public health organizations continue to explore how multi-sector data sharing and collaboration can help improve community health. Collaborating with community, government, and health organizations, HIEs are partnering on the development of “use cases”, shared governance, and data-informed interventions to improve health. Join us as SHIEC celbrates U.S. National Health IT Week October 8th thru the 12th.
